I have ALMOST the same issue with this phone that the 6700/6800 had but on this phone the phone wakes up, shows incomming call, then finally rings, this is 3+ rings on the other end.
I have a MP3 cut ringtone less than 25k in the windows rings folder, not on the storage partition.

I have some contacts, not too many (140) with possbily 15 of them with 3 numbers, the same as on my mogule 6800, but on it there was nowhere near this long of delay.
Exact happenings.
caller dials 1 ring, phone wakes, light starts to flash, ring 2, caller id comes up, ring 3, phone starts to vibrate and ring, at this point I can finally answer.
This happens in all power modes, sleep, on screen dim, on and useing.

The new phonecanvas.dll in both the Diamond & Pro take forever to load & are causing the problem you describe. People have reported the delay goes away if you hack the registry to disable the HTC phone skin, but unfortunately doing so causes other problems, such as the bluetooth icon disappearing, the vision arrows staying solid white, and the volume slider not working. Hopefully HTC will release an update to fix the lag in the dialer to improve this problem.
The registry key is HKLM\security\phone\skin\enabled & the value is 0 for off and 1 for on.
Marc's correct that having alot of contacts probably exasperates the problem.
Disabling the skin completely does cause issues.. try this instead:
The HTC Dialer Skin on the is pretty.... slow to load, at least that's the consensus. Skywing on xda-developers.com give us the following registry tweak:
In:
HKEY_LOCAL_MACHINE\Security\Phone\Skin
Set ext to \windows\PhoneCanvas.dll
Rename the DIALER/PROGRESS folders, possibly: oldDIALER/oldPROGRESS
This will give you the default WM Dialer Skin, which appears to load faster.
