This really depends on how much use you're planning on. If it's going to be low, roaming may be an OK alternative. If you search the web for "International SIM" or "Italian SIM" you'll find costs.

Does TeleNav even work in Italy? I'd check before counting on it being available.
For Europe, you might want to check out Route 66. They're a European company that's been around a while and has software that covers the area quite well. You install it on your memory card from a DVD, then activate it, so it goes where you want it. I like their software, but didn't like the US POI database much. There's also iGuidance and iGo (both of which I've heard good things about).
