I thinka Nutshell with a skin case would work. But I am also looking at flip lid cases with belt clips. Some of the sideways holster cases look good too.

Again, I am very interested in your research since you're doing exactly what I am looking for. Thanks, and keep up the good work!
Those 6 things are exactly what I was looking for. But if you want to remove the phone from a clip and not remove the phone from the case that means you would have to use a lock/release clip and all lock release clips I've seen protrude except for Krusells belt adapter which fits any case with a krusell multi adapt, but every case I've ever seen with a multiadapt has plastic over either the screen or keyboard or both and you said you also wanted to be able to use your case without a belt its no good.

With a flip lid case there is going to be even more case on top of that an inch of clip and an inch of treo to stick out.

So there are no low profile lock releases. There are many low profile cases in which you remove the phone from the case. There are a few low profile cases that you use the phone in the case such as Codi sport , which says their Treo 600 case will work for the 650. Heres a better picture . But then you have to unwedge the rigid clip from your hip every time you want to use it. The lowest profile lock release clip I have ever seen was the UniQase , but then your not realy protecting your phone since it faces out with no cover on the screen. And Its not 100% open access to the buttons. I bought one of those an I couldnt stan pushing the buttons through the silicon. I mailed it to the first person to send me 2 bucks in stamps.

I looked for 4 months for what your looking for and never found it. I ended up going with the basic needs (well made, low profile, easy to get phone in and out) and put aside the fear of an unprotected phone out of the case.

It looks like you have to accept a protuding case or put aside the fear of an unprotected phone out of the case or deal with slightly covered buttons or deal with an "in the case use case" that is hard to take on and off.
